PERSON CENTRED PLANNING
Crest Support Services is committed to providing quality services through person centred planning. The Crest has incorporated Personal Outcome Measures (Outcomes) as the quality enhancement tool for ensuring supports and services are responsive to and focused on the individual.
Crest Support Services has been working with The Council on Quality and Leadership (CQL), an external accrediting organization, to implement Personal Outcome Measures, and ensure continuous quality enhancement of services.
The seven areas of focus for Personal Outcomes Measures are:
